If your healthcare provider has created a Teleon app account for you and sent you an email or SMS invitation, then please follow the steps below to complete the signup process:
1. Click on the Sign Up link in the email or SMS invite you received. Teleon will automatically send you a 6-digit code via email or SMS.
2. On the Verification Code page, enter the 6-digit code you received and click the Continue button. If you did not receive a code, then click the Resend Code link on this page.
3. On the Action Required page, create a new password by entering it in the Password field. You will also be able to view your Teleon username on this page.
4. Once you click the Complete button, you will be logged into your account on Teleon’s web application. You can also download Teleon’s iOS or Android app on your mobile device by using the links below:
