With Teleon Secure Forms, you can create forms with a signature field/component for various purposes (e.g., patient registration form, consent form).
1. From the Create Form page, open the Advanced tab from the left-hand menu of components.
2. Drag and drop the Signature component onto the center canvas.
3. Design the rest of your form and click the Save button at the top right of the page. You can send this form to your recipients via email/text or using the Web Link option in the Send Form page.
Obtaining Signatures via Web Link Forms
If recipients sign a form that was shared using a Web Link, then Teleon will require their name and email address in order to confirm their signature and send them a PDF copy of the completed form.

For all signed forms, Teleon will automatically send a PDF copy of the completed form to the recipients via their email address.
If your form has passcode activated, then your recipients will be required to enter a 6-digit passcode to access the PDF document. Teleon will automatically send this passcode to a recipient via email after the recipient clicks on the PDF link in the email.
